What Is Chronic Care Management?

If you have Medicare or both Medicare and Medicaid, and have two or more chronic conditions, Medicare is offering CCM services to help you manage your health and spend more time doing the things you enjoy, in good health. If you have more than one chronic condition like Alzheimer’s disease and related dementia, arthritis, asthma, atrial fibrillation, autism spectrum disorders, cancer, cardiovascular disease, chronic obstructive pulmonary disease, depression, diabetes, hypertension, substance use disorder, or, and infectious diseases such as HIV/AIDS, CCM is an important piece of the care that we can offer.

Benefits of Chronic Care Management

  • Regular CCM means you can better manage your care and spend more time focusing on your health. CCM can help you work toward your health and quality of life goals. Better care management can help you avoid health events such as trips to the emergency department, a fall, or worsening health.


  • Coordinated care means you will get personal attention and help from a health care provider you know and who knows about your health conditions and helps to keep you healthy.

Services Include:

  • At least 20 minutes a month of chronic care management service
  • Personalized assistance from a dedicated health care professional who will work with you to create your care plan
  • Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more
  • Phone check-ins between visits to keep you on track
  • 24/7 emergency access to a health care professional
  • Expert assistance with setting and meeting your health goals

What are some examples of services I receive under this program?

The goal of this program is to improve overall health and quality of life by closing any gaps in care. Some of the services you will receive will therefore include:

  • Assistance with medical equipment, education support on your treatment plan, training you need to carry out health-related tasks, being available to answer questions about your health, etc.
  • Managing your referrals to other providers.
  • Managing your prescriptions,
  • Monitoring of your general health status, such as with weight and blood pressure checks, blood sugar control and other variables.



How will I know if I qualify for this service?


According to Medicare, you or your loved one qualify if you have Medicare and have 2 or more chronic illnesses. We check your qualification before beginning service. You also qualify if you have only 1 chronic condition under another program called Principal Care Management


Do I have to pay anything upfront?

No. Everything is paid for by Medicare. You are responsible for a cost-sharing, if you do not have a secondary insurance.
